JOB SUMMARY
Under the direction of the EMP Operational Director, the RNCB-NSWOC guides the wound, ostomy and continence care management for patients with complex needs at the unit and regional levels by ensuring care according to best practices and consistent with the EMP Wound Care Framework.
Acts as a clinical leader, role model, change agent and provides guidance by promoting best practices, centered on the patient and in support of a collaborative care model.
Supports and facilitates a collaborative approach with the interdisciplinary team members to assess and develop a comprehensive wound, ostomy and continence care plan to optimize patients’ outcomes.
Contributes to quality improvement initiatives and actions as required and supports the design and revision of initiatives / services.
ROLES AND RESPONSIBILITIES
Clinical Leadership and Patient Care
- Provides clinical leadership and guidance to the local wound care teams, applying advanced knowledge, skills, and judgment to manage complex patient issues in wound, ostomy, and continence care.
- Practices within RNCB scope and respects the scope of other health care team members.
- Delivers advanced wound, ostomy, and continence care based on best practice guidelines and current evidence when required.
- Conducts comprehensive baseline and follow-up assessments for patients with complex wounds, ostomy, or continence needs and guides the EMP clinicians in planning and adopting optimal intervention plans.
- Guides and supports the EMP clinicians in the identification wound classification and etiology, and selecting the appropriate products based on wound type, goals of care, and follow-up requirements.
- Supports the coordination of care for complex wound cases, including referrals to interdisciplinary team members and external partners when necessary.
- Assesses factors that impede healing (e.g., perfusion, glycemic control, nutrition, comorbidities, medications, tobacco use, infection) and initiates appropriate interventions.
- Guides and supports the care teams in the development and documentation of detailed care plans with time-specific goals in collaboration with EMP clinicians as well as patients and families, empowering them to participate in care.
- Provides education and knowledge transfer to health care teams, patients, and families on wound, ostomy
- Establishes clear communication strategies with wound care teams, physicians, and other team members to ensure continuity of care.
- Collaborates with other NSWOCs, RNCCs within EMP and across health networks to maintain consistency and share best practices.
Staff Orientation and Professional Development
- Participates in educational sessions and disseminates relevant information to clinical staff.

- Assists with identification of educational needs, training needs and opportunities.
- Guides and collaborates with the development of patient’s educational tools and resources required for patients and families’ teaching.
- Supports EMP accountability framework to contribute to efficient performance.
Quality Improvement and Risk Management
- Collaborates in quality improvement initiatives as required.
- Participates in quality improvement and risk management activities by methods such as evaluating effect of changes in practice, generating recommendations for alternative approaches to practices, conducting safety audits, and supporting implementation of teaching/learning when required.
- Supports EMP accountability framework to contribute to efficient Key Performance Indicators.
Research
- Assists in finding potential patients for researchers through the promotion of approved projects.
- Participates in research projects as required.
- Supports and leads clinical trials at the unit and regional levels.
REQUIRED QUALIFICATIONS AND OTHER JOB REQUIREMENTS:
- Bachelor’s degree in nursing;
- Actively registered or ability to register with the New Brunswick College of Nurses.
- CNA Certification – Nurse Specialized in Wounds, Ostomies and Continence (NSWOC).
- Minimum of five (5) years of experience with the Extra-Mural Program acquired over the past eight (8) years.
- Current CPR Level C certification that has been issued in the last 12 months
- Demonstrated ability to provide education programs using exceptional interpersonal, leadership, and teaching skills.
- Demonstrated knowledge of current health trends, principles of home health care, community care, senior, acute, supportive and palliative care;
- Knowledge of the target population of EMP and available community resources;
- A good driving record with a valid New Brunswick Class 5 Driver's License;
- Demonstrated competency in Microsoft office (Word, Excel, Power Point) as well as confidence in computer use.
- Strong computer skills and knowledge of Microsoft Office (Word, Excel, Power Point).
- Ability to develop effective partnerships with the community and hospital network.
- Commitment to ongoing nursing learning, training and professional development.
- Ability to work efficiently as a member of the team and possess leadership and coaching skills, strong interpersonal skills, good judgment and decision-making skills;
- Must demonstrate capacity to work independently;
- Able to work all shifts as assigned, maintaining a good attendance record;
- A high regard for confidentiality; and
- Submit a valid and clean criminal record check.
