The most efficient way to submit a request for services, is through our Online Referral/Request for Service Form. (Note: Ensure your browser is set to allow pop-ups.) A Reference Number will be provided upon completion.
ACCESS THE ONLINE REFERRAL / REQUEST FOR SERVICE FORMA request for service form can be submitted by an individual, family, substitute decision maker or any health care provider to determine if the person is eligible for service with the Extra-mural program. All requests for service will be managed through the care coordination center (CCC) if you do not hear from the CCC within 2 business days , please call 1-(844) 982-7367.
Alternative methods for submitting a referral / request for services form are:

Who is eligible for EMP services?
People living within New Brunswick with a valid Medicare card (or in the process of receiving one) are eligible to receive Extra-Mural services, as long as health care needs can be met safely in the home.
- Eligibility for admission is based on the following:
- The individual has an identified healthcare need;
- The individual has a need that requires the provision of healthcare services in their place of residence or designated community setting;
- The individual’s environment is assessed as suitable and safe for both the individual and the healthcare provider;
- The availability of resources for the provision of safe and effective home care;
- The individual has an attending physician or Nurse Practitioner (NP);
- The attending physician must have admitting privileges within the province of New Brunswick;
- The patient’s NP must be an employee of a New Brunswick Regional Health Authority (excluding those NPs whose role is as an emergency room NP);
- The individual accepts the services offered to him and is aware of his rights and responsibilities as defined in the EMP patient guide.
Note: For patients who require admission to the EMP who do not have a primary care provider, EMP service providers, with the patient’s consent, will collaborate to facilitate access to a Physician/NP and/or with NB Health link (if already established in the health zone where the patient lives) to act as the attending Physician/NP for the EMP admission.

What are your hours of operation?
If you are submitting the Referral Form after 4 pm during the weekdays, during the weekend or during a statutory holiday and the health need is URGENT, please call EMP at 1 (844) 982-7367 and EMP’s answering service will direct your call to the local EMP unit. Urgent referrals are for services that require services within the next 24 hours or individual would have to be transported to local Emergency room for care.

Do I receive a confirmation/reference number when sending a request for EMP services form?
Yes, after you successfully submit your EMP Referral Request Form, you will be given a Reference number. Applications for EMP services is managed by the Care Coordination Center (CCC). If you do not hear from the CCC within 2 workdays, please call 1-844-982-7367.

Do I need a primary physician to access EMP services? If I do not have one, what can I do?
For patients who require admission to the Extra-Mural Program who do not have a primary care provider, EMP service providers – with the patient’s consent – will facilitate access to a primary care provider to act as the attending physician or nurse practitioner for the EMP admission.

I do not have a valid New Brunswick Medicare Card, or I live out of Province and require EMP Services while I am in New Brunswick, what should I do?
New Brunswick residents and non-residents without a valid NB Medicare card and not in the process of receiving a card shall also be eligible to request services. The individuals shall meet the eligibility and admission criteria and agree to assume responsibility for the full cost of the services provided.
