Imagine a career where, at the end of every day, you know you've made a positive difference in the lives of people who need your help.
The New Brunswick Extra-Mural Program (EMP), often known as the “hospital without walls,” has operated under the simple philosophy of helping New Brunswickers receive the care they need in the places where they are healthiest and happiest - their homes and communities. EMP makes a difference in the lives of those it serves by drastically reducing hospital admissions and greatly increasing patient satisfaction with its level of health care.
As part of EMP, your role is vital in our province's overall health. Join an integrated, multi-disciplinary team of health care professionals, dedicated to providing New Brunswickers with outstanding, highly coordinated primary health care.
JOB SUMMARY
The Extra-Mural Program Hospital Liaison Nurse is an integral member of the interdisciplinary team, responsible for coordinating care between hospital and home at both the local and provincial levels. This includes completing a bedside assessment of each patient’s medical, functional, and social needs to support safe, effective, and timely discharge planning.
Key responsibilities include coordinating EMP clinical services, reducing unnecessary hospital admissions through proactive care planning, and educating hospital personnel about the scope and limitations of the EMP. The Liaison Nurse also engages patients and caregivers in developing care plans, provides information about EMP services, and assists with short-term equipment needs within program guidelines.
By offering leadership, coordination, and clear communication across care settings, the Liaison Nurse helps ensure patients receive the right care, in the right place, at the right time.
