JOB SUMMARY
Under the direction of the EMP Operational Director, the Wound Care Coordinator Nurse supports and oversees the EMP provincial Wound Care Framework providing a comprehensive wound care management for patients with complex wounds by ensuring a standardized approach to care and consistent expertise across the province.
Acts as a clinical leader, role model, change agent and provides guidance by promoting best practices, centered on the patient and in support of a collaborative care model.
Supports and facilitates a collaborative approach between the Wound Care Team members and the interdisciplinary team members to assess and develop a comprehensive wound care plan to optimize patients’ outcomes.
Support the development and the implementation of a product and supplies management process specific to wound care ensuring a provincial approach in the evaluation and selection process.
Contributes to quality improvement initiatives and actions as required and supports the design and revision of initiatives / services as well as the implementation of initiatives / services at the regional and provincial level.
ROLES AND RESPONSIBILITIES
Clinical Leadership and Patient Care
- Supports quality patient centered care and excellence in practice by promoting interdisciplinary collaboration in wound care management.
- Acts as provincial resource for the EMP Wound Care Framework, ensuring wound care teams engage in best clinical practices.
- Collaborates with internal and external partners, participates in a variety of designated professional or operational committees at the regional and provincial level.
- Ensures that the wound care management process is effective, efficient, and consistent across the province.
- Provide guidance and support to enhance wound care skills and knowledge of the wound care teams.
- Supports the adoption and implementation of clinical protocols, best practices, and policies in relation to wound care.
- Help ensure that the teams adhere to evidence-based guidelines for wound assessment, treatment, and monitoring.
- Tracks the success of wound care interventions by reviewing patient outcomes (wound healing rates, complications and patient satisfaction), using data to assess the effectiveness of care and identify areas for improvement.
- Promotes interdisciplinary patient centered care approach and maintains open lines of communication between members of the interdisciplinary team.
- Promotes guiding principles of home care practice in wound care with clinical staff aligning with Home Care Lens and supports them in acquiring skills to be competent.
- Collaborates in the implementation of appropriate home care indicators and ensures that EMP staff is aware of the indicators and follows the provincial EMP Clinical Practice Leadership documents for home care.
- Coordinates the wound care product and supplies review and selection process.
- Ensures the EMP wound care products and supplies catalogue (ReqLogic) is accurate and up to date.
Staff Orientation and Professional Development
- Identifies wound care learning requirements, in conjunction with EMP leaders and Skin and Wound CPLC, provides input into the development of learning objectives; provides clinical support and guidance and ensure the effectiveness of learning opportunities are evaluated.
- Provides input into the development of a comprehensive wound care orientation program as per the EMP Wound Care Framework, in collaboration with the Skin and Wound CPLC.
- Assists with identification of educational needs, training needs and opportunities.
- Guides and collaborates with the development of patient’s educational tools and resources required for patients and families’ teaching.
- Supports EMP accountability framework to contribute to efficient performance.
Quality Improvement and Risk Management
- Collaborates in quality improvement initiatives as required.
- Participates in quality improvement and risk management activities by methods such as evaluating effect of changes in practice, generating recommendations for alternative approaches to practices, conducting safety audits, and supporting implementation of teaching/learning when required.
- Supports EMP accountability framework to contribute to efficient Key Performance Indicators.
Research
- Assists in finding potential patients for researchers through the promotion of approved projects.
- Participates in research projects as required.
REQUIRED QUALIFICATIONS AND OTHER JOB REQUIREMENTS:
- Bachelor’s degree in nursing;
- Actively registered or ability to register with the Nurses Association of New Brunswick;
- Minimum of five (5) years of experience with the Extra-Mural Program acquired over the past eight (8) years;
- Demonstrated ability to provide education programs using exceptional interpersonal, leadership, and teaching skills;
- Demonstrated knowledge of current health trends, principles of home health care, community care, senior, acute, supportive and palliative care;
- Knowledge of the target population of EMP and available community resources;
- A good driving record with a valid New Brunswick Class 5 Driver's License;
- Have a current CPR Level C certification or be prepared to obtain within 3 months;
- Demonstrated competency in Microsoft office (Word, Excel, Power Point) as well as confidence in computer use;
- Ability to work efficiently as a member of the team and possess leadership and coaching skills, strong interpersonal skills, good judgment and decision-making skills;
- Must demonstrate capacity to work independently;
- Able to work all shifts as assigned, maintaining a good attendance record;
- A high regard for confidentiality; and
- Submit a valid and clean criminal record check.
