JOB SUMMARY

As part of the management for the Care Coordination Centre, you will provide leadership to the team of Liaisons to support appropriate and timely access for people to the Extra Mural program and to support transitions during their care journey. You will assist with standardizing practices, including the integration and use of the e-health solution. You will be an ambassador to hospital and community partners to inform and promote the philosophy, objectives and policies of the Extra-Mural Program.

QUALIFICATIONS:

  • A minimum of five years of experience in EMP
  • Can demonstrate experience in understanding a geographically dispersed, complex, multi program/service environment.
  • Can effectively communicate knowledge to professionals to better support patients;
  • Foster an environment that supports teamwork and collaborative environments with hospital/community partners;
  • collaboration and good communication with all EMP Managers regarding local issues or unique situations
  • Conduct real-time assessments and audits to monitor care provisions and clinical outcome measures and indicators;
  • Have the ability to exercise independent judgment and can problem solve with discretion;
  • Are committed to providing stellar levels of customer/patient service and have a passion for growth within EM/ANB Inc.;
  • Possess and demonstrate leadership abilities with a proven track record for inspiring and motivating others;
  • Are skilled in leading and managing change among the team and partners
  • Are skilled in leading and developing staff;
  • Use a positive and creative approach to problem solving;
  • Possess outstanding communication, , and dispute resolution skills;
  • Travel may be required;
  • Excellent work performance and attendance at work; and
  • Have working knowledge of standard business computer programs (ie. Microsoft Office Suite
